Required Application Materials:
- Completed Application
- Color copy of Drivers License
- 2" x 2" front face color photograph
- Certificate of General Liability insurance for business (as listed on the first page checklist).
- Current schedule of fees charged by applicant (as listed on the first page checklist).
- Copy of the notice that will be attached to immobilized vehicles
You will have your fingerprints taken at the time you submit your completed application. We do not accept fingerprint cards and are unable to access fingerprints taken by other agencies..If you submit your application materials via email, we will contact you to arrange a time to complete fingerprinting.
