We accept passport applications by appointment only.

The City of Meridian Clerk’s office is a passport application acceptance facility. We accept applications for new passports on behalf of the U.S. Department of State. We process the first portion of your application and then mail it to the U.S. Department of State. The State Department then will review and finish processing your passport.

We accept passport applications by appointment only between the hours of 8:00 am to 3:30 pm, Monday through Friday.

Passport fees are only payable by check, money order or cashier’s check. The U.S. Department of State is unable to accept cash or credit/debit card.

Each individual applying for a passport must be present at the appointment, including children. Children under the age of 16 must have consent from both parents. If only one parent is able to appear, a notarized consent form, DS-3053 is required in place of that absent parent. We highly recommend you visit the website travel.state.gov for all the required documents you might need for your appointment.

You Must Apply in Person If:

  • You are applying for your first U.S. passport
  • You are under 16 years of age
  • Your previous passport was issued when you were under the age of 16
  • Your previous U.S. passport was lost, stolen or damaged
  • Your name has changed since your U.S. passport was issued and you are unable to legally document your name change
  • Your previous U.S. passport was issued more than 15 years ago

U.S. CITIZENSHIP:

When applying for a U.S. Passport, you will need to have a certified birth certificate issued by the city, county or state. A certified birth certificate has a registrar’s raised, embossed, impressed or multicolored seal, registrar’s signature and the date the certificate was filed with the registrar’s office. We are not able to accept hospital certificates or birth abstracts.

PASSPORT PHOTOS:

Your passport photo is a vital part of your application. Your photo needs to be in color and taken within the past 6 months. You will need a 2”x2” with a full-face, front view (head should measure between 1 1/8” and 1 1/3”) with a plain white or off-white background. We recommend you use one of the many retail photo services that provide passport photos. We do not offer passport photo services at our facility.

EVIDENCE OF IDENTITY:

When applying for a U.S. passport, acceptable identification must be presented at the time of application.

The form of I.D. used must be valid and bear a picture of the holder.

A current driver’s license is the best form of I.D. Parents I.D. is used for minors. Minors between the age of 16-17 who have their own I.D. but are applying without a parent present should submit a written consent from a parent to obtain a passport.

FEES:

An Adult Passport Book fee is $130.00. It must be in check or money order made out to the U.S. Department of State. The passport card fee is $30.00.

You may purchase the card alone or with the passport book. It must be in check or money order made out to the U.S. Department of State. The execution fee is $35.00 in check or money order made out to the City of Meridian. (This is separate from the Passport fee). Adult passports are valid for 10 years from the issue date.

A Minor (15 years and under) Passport Book fee is $100.00. It must be in check or money order made out to the U.S. Department of State. The passport card fee is $15.00.

You may purchase the card alone or with the passport book. It must be in check or money order made out to the U.S. Department of State. The execution fee is $35.00 in check or money order made out to the City of Meridian (This is separate from the passport fee). Children’s passports are valid for 5 years from the issue date.
